Summary

The answer to the gap the 2012 finite-element paper left open. Patient-specific mitral valve modelling had been limited by the assumptions it had to make about constitutive material properties - so this paper estimates those properties from the images instead of assuming them.

The framework combines machine-learning image analysis with geometrical and biomechanical models. Chordae rest lengths and leaflet material properties are initialised with generic values and then optimised so that the simulated deformation between the open and closed valve matches what the 3D TEE images show, minimising the summed Euclidean distance to the image-derived closed geometry. Spatially varying leaflet parameters are estimated with an extended Kalman filter, exploiting the temporal information TEE provides.

Data Points

  • Material properties estimated from image data rather than assumed - the limitation the 2012 FE framework identified
  • Chordae rest lengths and leaflet properties optimised against image-derived open-to-closed deformation
  • Spatially varying leaflet parameters estimated with an extended Kalman filter over the TEE time series
  • Tested on 15 TEE acquisitions from 14 patients; simulated closure error 1.86 ± 1.24 mm point-to-point
  • Estimated parameters show the anterior leaflet is stiffer than the posterior, and that properties vary between individuals - consistent with the experimental literature
